Run this command in Terminal.app to remove duplicate items in your "Open With…" menu on OS X.

/System/Library/Frameworks/CoreServices.framework/Frameworks/LaunchServices.framework/Support/lsregister -kill -r -domain local -domain system -domain user

I've run this command almost weekly for months, it's safe and it works great.
