Under mcw-opioid-intake, "over the past month have you run out of your medication early" when you look at the results the answer "yes" (in this case shows) up in the row across from the "over the past month have you" Can you change it so that the answer is in the "run out of pain medication early" row
- Adult Intake with Headache pg 12. some weird formatting thing is going on near the right hand side under rehabilitative modalities
- Adult Intake with No head/neck pain- the headache questionnaires should not be showing up in this. The headache questionnaire should only show up when patient clicks any area on head/face. Same goes for the F/U survey.
This is because the patient had previously indicated as having head pain in the past, this is the way Stanford does it. Should we remove it?
- Speaking of headache questionnaire, can you change "how many headache free days did you have per month" to "how many days did you have a headache per month"
We can change it, but this is Stanford's, so it might be wise to keep it consistent with them.
- Adult f/u- Pg1.On the Body Map chart on the y-axis where it's labeled regions instead of having scales by increments of 15, can it just show the total #. For example, 8 regions were checked in the body map so it should just show the number 8 on the y axis.
This would require a relatively significant rework of the graphing, I don't really think it's an effective thing to focus on
- Adult f/u- Pg 6.mcw opioid question- after oxycodone there is a parentheses, can you move it so that its before roxicodone. There is also an extra space before Oxycontin
The spaces are because of the italics around the brand names. We can either remove the italics, or there will be extra spaces inside the parens, or an extra space before the comma after the parens
- Adult f/u-Pg 6. Mcw opioid follow up- the answer choice in this case "no" is in the row across from "over the past month have you". Can the answer choice be moved to the row "Taken you opioid medication on that the way it was prescribed."
- Also there's a typo in that last sentence it says "YOU" opioid medications. should be "your"
- Interventional procedures and Specialized pain management scales only showed up on the adult intake (with headache) survey but not the adult intake when with no head pain.
