You have to:

1. Migrate wordpress to Y! server normally(either into a dummy folder or re-name before #3)
2. Install Extension above
3. Install a new Blog from control panel into the desired folder you want your blog to reside on. (Re-name #1 if needed)
4. Re-name Y! installed blog you just created
5. Re-name Migrated folder from #1 to what you just created in #3
6. Delete folder from #4
